Other Reporting Options

Although the State and Alternative Fuel Provider Program strongly encourages fleets to use the online reporting system, the following options are available to those who prefer to report via e-mail or fax.

  • Spreadsheet and Instructions: State and Alternative Fuel Providers, Form DOE/FCVT/101 (Excel 24 KB)

    Download the Microsoft Excel spreadsheet, fill it out, save it to your computer, and e-mail it to afv_credit_acct@L-3com.com. Don't forget to attach the form to the e-mail before sending.

  • Annual AFV Reporting for State and Alternative Fuel Provider Fleets, Form DOE/FCVT/101 (PDF 479 KB). Download Adobe Reader

    Download the PDF, and read the instructions guidance that starts on page 6. Then fill out the reporting form, print it, and fax it to L-3 Communications at (703) 480-9359. If you have purchased and installed the full version of Adobe Acrobat, you may save the completed form on your hard drive and e-mail it to L-3 Communications at afv_credit_acct@L-3com.com.
